As a patient of Rekindling The Spirit Health Service we require you to provide us with your personal details and a full medical history so we may properly assess, diagnose, treat and be proactive in your health care needs.

We aim to protect the privacy and secure storage of your health information. You can request a copy of our privacy policy which includes information about the collection, use and disclosure of your health information.

We collect information that is necessary and relevant to provide you with medical care and treatment services, and manage our medical practice.

This information may include your name, address, date of birth, gender, health information, family history and contact details. This information may be stored on our computer medical records system and/or in hand written medical records.

Wherever practicable we will only collect information from you personally. However, we may also need to collect information from other sources such as treating specialists, radiologists, pathologists, hospitals and other health care providers.

We collect information in various ways, such as over the phone or in writing or in person. This information may be collected by medical and non-medical staff.

In emergency situations we may also need to collect information from your relatives or friends.

We may be required by law to retain medical records for certain periods of time depending on your age at the time we provide services.

We require your consent to collect personal information about you and to use the information you provide in the following ways:

  • Administrative purposes in running our medical practice.
  • Billing purposes, including compliance with Medicare and the Health insurance commission.
  • Disclosure to others included in your healthcare including treating doctors, allied health professionals and specialists outside of Rekindling The Spirit Health Service. This may occur in the form of referrals other doctors and in the reports and results returned to us following referrals.
  • Disclosure to other doctors in the practice and allied health professionals for the purpose of patient care and teaching.
  • To comply with any legislative or regulatory requirements e.g. notifiable diseases.
  • For reminder letters which may be sent to you regarding your health care and management.

You can decline to have your health information used in all or some of the ways outlined above but it may impact how we manage your health care to provide the best health outcome for you.
