ITC/CCSS
The Integrated Team Care (ITC) / Care Coordination and Supplementary Services (CCSS) is a program that links Aboriginal and/or Torres Strait Islander people to the health care providers they need for their chronic disease. The team of staff are made up of care Coordinators and Aboriginal Outreach Workers to support Aboriginal patients enrolled in the program. The team supports clients who are attending any of the following General Practices:
- Mainstream General Practices from Tweed Heads to Grafton
- Rekindling the Spirit Health Service “Jullums House
The program has eligibility criteria to be met, as well as a Doctors completing a Referral Form at your next appointment along with providing a copy of your care plan (a care plan may be called a GP Management Plan, Mental Health Care Plan or a Team Care Arrangement).
NOTE: Discuss with reception having a care plan done or reviewed so they can book a longer appointment, as this usually takes an hour to complete.
